The ethical landscape of providing mental health care to transgender/gender expansive (TGE) individuals is both intricate and crucial, demanding a heightened level of awareness and sensitivity from mental health/substance professionals. As TGE individuals increasingly seek behavioral health services, they often face unique challenges that can be exacerbated by societal stigma, discrimination, and a lack of understanding within the healthcare system. For behavioral health providers, this necessitates a thorough understanding of the ethical as well as clinical considerations that are specific to this population.
These considerations include navigating complex issues related to client autonomy, ensuring confidentiality, obtaining informed consent, and maintaining cultural competence. Moreover, the role of the behavioral health professional extends beyond the therapy room, encompassing advocacy for the rights and well-being of transgender clients in a broader societal context. Professional ethical guidelines, such as those provided by the American Counseling Association (ACA), the National Association for Addictions Counselors (NAADAC), the National Association of Social Workers (NASW), and the American Psychological Association (APA), offer crucial frameworks for addressing these issues. However, applying these guidelines in practice requires a nuanced understanding of the unique experiences and needs of transgender individuals.
The presenter will take you along on his own trans journey with storytelling and personal insights that highlight the everyday life and stresses for one transgender man who also happens to be a clinician and outspoken advocate for LGBTQIA2S+ inclusion in mental health and substance use treatment. You will be given a glimpse at a family narrative, everyday interactions with discrimination and fear, the masks and layers one wears for safety, and more. All of these are similar to issues that contribute to the mental health and wellness of our clients. Through this story and additional clinical insights, you will be able to better understand your clients' journeys and help them along. Behavioral health professionals must be vigilant in their efforts to avoid harm, promote autonomy, and ensure that their practices are both affirming and supportive of the diverse identities within the greater transgender community while adhering to both legal landscape and professional ethics.
Learning Objectives:
- Identify and articulate at least three key ethical guidelines from the ACA, NAADAC, NASW, and the APA related to the mental health care of transgender clients.
- List and explain at least 5 strategies for providing gender-affirming care that align with ethical standards for working with transgender clients.
- Develop a much greater and more intimate understanding of clients with/of transgender experience to aid clinical conceptualization and clinical approach for TGE clients.

Gus Raymond, LMHC, CADC, NCC
Gus is a certified alcohol and drug counselor practicing in the rural northwest corner of the state of Iowa. He currently works as a mental health therapist and substance counselor in private practice. He completed his master's in clinical mental health from Adler University in Chicago, IL., and is currently a Ph.D. student in Sociology at the University of Iowa pursuing research related to cultural and community support for marginalized populations.
Gus specializes in marginalized populations, particularly LGBTQ+ and immigrant/refugee communities. As a multi-racial transgender man, he is no stranger to diversity and forms of oppression, particularly in the world of education and mental health services. Gus also has lived experience with substance use disorder, from which he celebrates nearly two decades of recovery. He offers professional development training in restorative practices that are equitable and just, workshops on cultural humility and education, and working with multicultural youth experiencing substance and mental health difficulties.
