Hospice behavioral specialists are considered the number one patient and family advocate for patient care and end-of-life wishes. With the evolution of telehealth, it's particularly important to ensure that behavioral health specialists are equipped to set up and conduct an initial and ongoing assessment for hospice patients and their families. When a patient enters hospice care, it is imperative that they have access and support to their behavior health specialist for end-of-life planning, community resources, emotional support, crisis intervention, legalities, psychoeducation on the dying process, prevention resources for caregiver burnout, funeral planning, and as needed placement information in their community.
In this course, the hospice behavioral health specialist will be able to identify and explain the valuable role of the behavioral health specialist in hospice care, provide discussion on what needs and end of life wishes need to be assessed at the initial biopsychosocial assessment and ongoing assessment, what technology is needed to ensure accessible and successful telehealth visits, and explain applicable laws and legalities for hospice patients and their families. This course will include valuable tips and resources for behavioral health specialists in the hospice care role.

This course goes in-depth into the legal role of the hospice and palliative care behavioral specialist. The behavioral health specialist plays a vital role in providing psychoeducation and assisting patients and their families regarding the legal paperwork that needs to be completed for end-of-life wishes and care. This course will focus on four core components of end-of-life care: Medical Power of Attorney (POA), Funeral Designation Form and Funeral planning, State Medicaid application process, and pre-placement procedures for State Nursing Home Placement.
Medical POA planning is imperative for end-of-life care to ensure that the patients' end-of-life wishes are followed. This course will instruct the learner on how to engage the client and their family in conversation regarding end-of-life medical wishes and how to complete legal POA paperwork. Funeral planning is also a core component for the hospice and palliative behavioral health care specialist to assess funeral arrangements and engage the client and family in funeral planning. A portion of hospice patients will need placement outside of their family home as their terminal illness progresses. This course will explain and explore the different placement options and how to complete pre-placement paperwork for State Nursing Homes. A portion of patients and families will have financial burdens, which makes applying for Medicaid imperative. This course will provide psychoeducation and an overview of the Medicaid eligibility criteria and instruct the learner on how to apply for State Medicaid.
- Explain the process of setting up and implementing Medical Power of Attorney for hospice patients.
- Explain the process of setting up and implementing Funeral Designation forms for hospice patients.
- Explain the nursing home pre-placement process and explain how to complete the required documentation.
- Discuss the requirements of mandated reporting regarding vulnerable adults.
